I don’t know about you, but I have a hard time remembering when my family members go to the doctor, dentist, or eye doctor and for what reason each time.
However, keeping track of this information can be helpful to a) be sure you are visiting the doctor frequently enough/on a regular schedule, b) find any patterns to illnesses, and c) have information available to share among all your doctors if you have to see specialists, etc.
This printable is meant to be printed as one sheet for each family member. You can record the date of your appointment, which doctor (or dentist, lab, etc.) you saw, the reason for your visit or your doctor’s diagnosis, and what treatment plan you discussed.
