I put mine in a sheet protector. The information that does not change is already filled in – our cell numbers, address, phone number, children’s names, etc.
Then, when we have a sitter, I can fill out any relevant information with a wet-erase marker. At the top, there is a space to record where you are going and what time you will be home.
I have also included a space to write your home address and phone number. If there were an emergency, not all babysitters would know your exact address off the top of their head, and this could waste valuable time.
There is space for three children on this sheet. Your children’s names go in the pink boxes in the middle of the sheet. If you have more than three children, you will have to print a second sheet.
